ELIGIBILITY PATHWAYS:

Pathway I - Acute Care CPNP Exam - Acute Care PNP graduate within the past 24 months.
Pathway II - Acute Care CPNP Exam- Acute Care graduate more than 24 months ago.

DOCUMENTATION- Please read each item carefully to see what is required for your pathway: Any required forms are in PDF format and must be printed out, completed and mailed to the PNCB.

• DOCUMENTATION OF CURRENT PALS for Pathways I and II. Please submit a copy of both the front and back of your current PALS card.
• TRANSCRIPTS for Pathway I and II. Final MSN or MSN and Post Master's transcript(s) must be mailed directly from the educational institution to the PNCB office.
• DOCUMENTATION OF ACUTE CARE EDUCATION FORM for Pathway I and II.
• ACUTE CARE SKILLS VERIFICATION FORM for Pathway II.
• DOCUMENTATION OF CLINICAL PRACTICE HOURS FORM for Pathway II.

All exam results are mailed USPS First Class.
