Please note: For timely processing of your application, please be sure to mark your exam type on the application form and include appropriate payment. You will select from one of the following:

A. On-site Exam in Paper-Pencil Format - offered at conferences and hospital sites. Please be sure to identify the hospital/conference site and date on the application.

B. Exam by Computer-based Testing (CBT) - offered year-round at over 280 Prometric testing centers across the US.
