Apply online for your exam and arrange for the appropriate supporting documents below to be completed and sent.

To review the purpose and proper submission of these forms, please refer to your eligibility requirements: Pathway I or Pathway II.

• Documentation of Education Form - To be signed and submitted by your program's designated signer(s).
• Transcripts - Arrange to have final transcript(s) sent from your institution. Full details on transcripts are available from your eligibility webpages: Pathway I or Pathway II.
• PALS Documentation - Please fax both sides of your current PALS card to 301-330-1504.

• Skills Verification Forms - Two copies of this updated form must be signed by physician and/or NP colleagues. Previously submitted forms cannot be accepted.
